The Court finds that the ward herein has established or re-established his or her residence, or a legal
settlement in Butler County; that a guardianship was instituted for him or her in another county within the
state of Ohio; that said county desires to transfer jurisdiction of this guardianship to Butler County; and that
such transfer of jurisdiction would be in the best interest of the ward. Therefore,
The Butler County Probate Court hereby accepts jurisdiction over the ward and guardian in this
matter.
