(Name of person), the Alleged Victim in the above-captioned case, moves the
court to issue a protection order containing terms designed to ensure the safety and protection of the alleged
victim in this case, pursuant to its authority to issue a protection order under section 2903.213 of the Ohio
Revised Code.
A complaint, a copy of which has been attached to this motion, has been filed in this court charging the named
defendant with a violation of section 2903.11 (Felonious Assault), 2903.12 (Aggravated Assault), 2903.13
(Assault), 2903.21 (Aggravated Menacing), 2903.211 (Menacing by Stalking), 2903.22 (Menacing), or 2911.211
(Aggravated Trespass) of the Ohio Revised Code or a violation of a municipal ordinance substantially similar to
2903.13, 2903.21, 2903.211, 2903.22, or 2911.211 of the Ohio Revised Code.
I understand that I must appear before the court, at a time set by the court not later than the next day that the
court is in session after the filing of this motion, for a hearing on the motion.
I also understand that any protection order granted pursuant to this motion is a pretrial condition of release and is
effective only until the disposition of the criminal proceeding arising out of the attached complaint or until the
issuance under section 2903.214 of the Ohio Revised Code of a Civil Stalking Protection Order arising out of the
same activities as those that were the basis in the attached complaint.

(2) By its own definitions [see R.C. 2903.213(A)], this statute does not apply to a complaint that
involves a person who is a family or household member. In those cases where the Alleged
Victim is a family or household member of the defendant, use Criminal Temporary Protection
Order (TPO) forms and procedures under R.C. 2919.26, and/or Civil Protection Order (CPO)
forms and procedures under R.C. 3113.31.
