violence or child abuse therefore objects to the Court mailing a copy of this Notice
to the Non-Residential Parent AND REQUEST THAT THIS MATTER BE SET FOR HEARING on the
issue of whether a copy of said Relocation Notice should be mailed to the NonResidential Parent as set forth in ORC 3109.051(G)(2).

in the best interest of the child(ren) to revise the Parenting Time Schedule in this
