THE APPOINTMENT OF A GUARDIAN FOR HEARING
This case is before this Court on an application to dispense with the appointment of a
guardian of the estate for a minor.
The Court finds that all of the minor’s parents or legal custodians have not consented to this
application.
Wherefore, it is the order of the Court that the application to dispense with the appointment
of a guardian of the estate of the minor be set for hearing before this Court on the ____ day of
__________________, 200__ at ______ o’clock ____.m. This hearing will be held before
Magistrate ________________, Montgomery County Probate Court, 41 N. Perry Street, 2nd
Floor, Dayton, Ohio 45402.
The applicant is ORDERED to give written notice of the hearing on this application, by
certified mail, to the minor’s parents and/or legal custodians who have not consented to this
application. The applicant shall file proof of service of notice of hearing with the Court prior to
the above hearing date.
