The undersigned (hereinafter Probationer) does hereby acknowledge that he/she, during the continuance of his/her
probation, shall observe and comply with the terms and conditions set forth below:

ordered and/or approved by the Warren County Juvenile Court or the Warren County Juvenile Court Probation Department,
including, but not limited to, compliance with all rules, regulations, treatment contracts and recommendations of treatment
providers.
2. Probationer shall not view or possess any pornographic or sexually explicit material or things that are primarily used for sexual
purposes. “Pornographic material, sexually explicit material, or things that are primarily used for sexual purposes” include
printed materials, whether text or pictures, video tapes, DVDs, films, internet websites and other similar materials viewed/read
via computer. Probationer shall not view or possess any material or thing in any instance in which it is uncertain as to the
nature of materials as pornographic, sexually explicit, or as a thing that is primarily used for sexual purposes unless the Court or
Probation Officer authorized Probationer to view or possess such material or thing.
3. Probationer shall not have contact/communication, direct or indirect, with the victim of the offense unless approved by
Probation Officer and then only to the extent and subject to such terms and conditions as may be approved by Probation Officer.
“Contact/communication, direct or indirect” as used herein means all forms and means of contact and communication,
including, without limitation, in person by word or gesture, by telephone, by email, by written correspondence or any of the
foregoing by and through another person.
4. Probationer shall submit to polygraphs as directed by the Probation Officer and/or the Court and answer truthfully all questions
posed to Probationer in the course of such polygraph examination. Unless the Court otherwise allocates the cost of the
polygraph examination, the Probationer shall be responsible for such cost. Payment is generally required at the time of the
polygraph examination.
5. Probationer shall not engage in romantic relationships or engage in any sexual conduct/contact without approval of the
Probation Officer.
6. Probationer shall not patronize any adult oriented business. An “adult oriented business” includes but is not limited to adult

guidance from the Probation Officer.
7. Probationer shall not have unsupervised contact with any minors, nor patronize and/or frequent any place where minors
congregate. Supervised contact is defined as direct supervision by an adult that has been approved by the Probation Department.
The adult will be aware of the probationer’s sexual offending conduct/history.
8. Probationer is not required to notify their Employer of their sexual offending history, but the Probation Department must
approve any employment prior to the probationer commencing any employment. The probationer may be subject to random
