I and my parent(s) / legal guardian(s) / custodian(s) will refrain from making unreasonable noise or an offensively coarse utterance, gesture or display or communicating unwarranted and grossly abusive language in all interactions with my Probation Officer and / or other court personnel. Probationers shall attend court hearings, probation meetings or other miscellaneous court / probation ordered appointments dressed in an appropriate and respectful manner. Failure to observe this rule may result in rescheduling of hearings, meetings and / or appointments. Repeated violations of this rule may result in the filing of a probation violation. I will attend school without unexcused absence or tardy. “Unexcused absence or tardy” means that an absence or tardy from school is not verified by a person other than my parent, guardian or custodian as being for a legitimate reason for absence or tardy or is not otherwise excused by an authorized school official. If absent from school for any reason, I understand that I am mot to leave my place of residence without my parents or have visitors until I have returned to school, I will report absences, tardies, detentions, Saturday schools suspension, expulsions or other school discipline. I will obey school rules and regulations. My curfew is p.m. on school nights and p.m. on non-school nights. House arrest until: ______________________________. I understand that my probation officer has the authority to change my curfew or return me to house arrest at any time by contacting me or my parent in person or by telephone. I will report any arrest, citation or a violation of the law or ordinance, or any school rule violations to my Probation Officer not later than the next business day. I will obey all federal, state and local laws. I will remain in Warren County unless I am given permission to leave by my parent/guardian. I will not spend the night away from my residence except in emergency or unless my parents are required to be away, and have pre-approved the plan with my Probation Officer and parent. I will not leave the state without permission from my Probation Officer. I will not possess, use, purchase or have under my control any harmful intoxicants, illegal substances, alcoholic beverage or other controlled substance, including any instrument, device or other object used to administer drugs or to prepare them for administration, except as lawfully prescribed for me by a licensed physician. I will submit to drug/alcohol testing if required by my Probation Officer. I will not purchase, possess, own, use or have under my control any firearms, deadly weapons, ammunition or dangerous ordnance. I will have no contact or communication with any person on probation, house arrest, aftercare, parole or under court supervision, or any person considered to be a negative influence by my parents and/or Probation Officer. I will comply with an order which may hereafter be made by my Probation Officer which establishes a curfew, places me on house arrest or restricts me from associating with certain persons or being at certain places. I will notify my Probation Officer immediately of any change of address, school, employment or phone number. I will pay all fines, costs and/or restitution as ordered. I will comply with all Court orders, including but not limited to, the following special terms of probation: _____________________________________________________________________________________________________________________ _____________________________________________________________________________________________________________________ _____________________________________________________________________________________________________________________ Revised Code section 2152.19(F)(1) provides, in pertinent part, that “During the period of a delinquent child’s community control … authorized probation officers who are engaged within the scope of their supervisory duties or responsibilities may search, with or without a warrant, the person of the delinquent child, the place of residence of the delinquent child, and a motor vehicle, another item of tangible or intangible personal property, or other real property in which the delinquent child has a right, title or interest or for which the delinquent child has the express or implied permission of a person with a right, title or interest to use, occupy or possess if the Probation Officers have reasonable grounds to believe that the delinquent child is not abiding by the law or otherwise is not complying with the conditions of the delinquent child’s community control.” _____________________________________________ Probationer __________________________________________________ Probation Officer (513) 695 - The undersigned parent(s)/guardian(s)/legal custodian(s) have reviewed the foregoing with the Probation Officer and acknowledge an understanding of the Probationer’s duties and responsibilities pursuant to these terms and conditions of probation and the Probation Officer’s supervisory authority. I/We further acknowledge my/our responsibilities to control any conduct or relationship of the Probationer that will tend to interfere with execution/compliance with these terms and conditions of probation. I/We further acknowledge our duty to refrain from any and all conduct which may obstruct or interfere with the Probation Officer’s supervision of the Probationer hereunder, including the Probation Officer’s right to conduct searches as outlined above.
