TRANSFER FUNDS AND CLOSE
CONSERVATORSHIP
_______________________________________ (“Parent”) moves the Court for an order
approving transfer of conservatorship funds to a Uniform Transfers to Minors Act (“UTMA”)
account and closing the conservatorship. Parent respectfully represents:
1.
The value of the assets of the conservatorship is: $_________________, which is less
than $4,000. A copy of the most recent account statement is attached.
2.
Parent wishes to transfer the entire assets of Account #_______________________
at: ____________________________________________(name of financial institution holding
conservatorship funds) to a UTMA account.
3.
Parent has created UTMA account #______________________ at _______________
_______________________(name of financial institution for UTMA account), with
________________________________(name of custodian) as custodian.
4.
Parent agrees that: (1) the funds are owned by Child, (2) parent will only withdraw
funds for Child’s benefit, and (3) Parent will not spend Child’s funds for any items which Parent
has a duty to provide for Child.
WHEREFORE, Parent prays for an order of this Court:

Authorizing the transfer of the conservatorship funds described above directly to the
UTMA account, within 30 days of the date of the order approving this motion.
2.
Ordering Parent to provide the Court with a receipt from the financial institution
