The above-named defendant makes this motion/petition to the court for an order extending the diversion period of
the driving under the influence of intoxicants (DUII) diversion agreement that this court entered in this. In

under ORS 813.250.
Fails to comply with the diversion agreement within the extended diversion period, the court will
enter a judgment of conviction on the DUII charge and will sentence the defendant.
