Respondent (full name of person restrained) This matter came before the Court on (date): The Protective Order is continued as set forth in the Order After Hearing. FIREARMS FINDINGS: (if the Protective Order is continued, modified, or renewed) Respondent represents a credible threat to the physical safety of the Petitioner or Petitioner222s children. At the time of the abuse, Petitioner was the husband, wife, or Registered Domestic Partner of Respondent (current or former) Or the parent of Respondent222s child (child must be born) Or living or had lived with Respondent in a sexually intimate relationship Or the child of Respondent or the child of an intimate partner* of Respondent. (*intimate partner means current or former husband, wife, or Registered Domestic Partner, current or former cohabitant, or the parent of Respondent222s child) The Order was issued after a hearing of which Respondent received actual notice and at which Respondent had the opportunity to participate. The Order restrains Respondent from harassing, stalking or threatening Petitioner or Petitioner222s or Respondent222s children or engaging in other conduct that would place Petitioner in reasonable fear of bodily injury to Petitioner or Petitioner222s or Respondent222s children. Respondent represents a credible threat to the physical safety of Petitioner or Petitioner222s children as found above. FIREARMS NOTIFICATION: As a result of this Order, it may be unlawful for Respondent to possess, receive, ship, transport or purchase a firearm or ammunition pursuant to federal law under 18 USC 247922 (g)(8) and state law under ORS 163.765(1)(b)(E) and ORS 166.250 to 166.270. This Order also may negatively affect Respondent222s ability to serve in the Armed Forces of the United States or to be employed in law enforcement. (42 USC 2473796gg(4)(e) requires this notice).
