COMES NOW the above-named Defendant, [ ] without counsel, [ ] by and

continuance of the case and for an Order Referring the Defendant for a Diagnostic
Evaluation for the following reasons:
1.
The wrongful conduct charged may be the result of
[ ] Alcoholism [ ] drug addiction for which the defendant may be in need
of treatment and unless treatment is received, the probability of future
recurrence is great.
2.
The defendant agrees to pay or arrange for the payment of the
costs of a diagnosis of the problem and treatment.
3.
The defendant waives the right to a speedy trial for an additional
90 day period commencing on the date the Order of Continuance is
entered.
The defendant understands that the treatment facility will forward the results
of the evaluation to the Department of Licensing, and that the Department of
Licensing may suspend the defendant’s driver’s license or privilege to drive until
such time as the defendant has enrolled or successfully completed an extensive
treatment program, whether or not the defendant elects to petition the court for a
Deferred Prosecution.
