NON-PARTICIPATING MANUFACTURER CERTIFICATION FORM
Attachment B
If you are currently certified in Wyoming, your brand(s) list as included in Wyoming’s Directory of
Certified Tobacco Product Manufacturers Their Brands and Brand Families is attached. Put a check
mark beside each brand, and each style that is being certified for sale in the State of Wyoming for the current
year, which answers question “1" below. Identify with an asterisk any brand sold in the state during the
preceding calendar year that is no longer being sold in the state as of the date of the Certification.
For new non-participating manufacturers that do not have The Directory page to use as a guideline,
please attach typed documents answering the following question.
1. List all brands, including styles and varieties, being certified for sale in the State of Wyoming, and
which will be included in The Directory’s June 1 release, or subsequent monthly updates if this Certification
is approved. Do not include a brand family in this Certification unless the entity certifying affirms that the
brand family is to be deemed to be its cigarettes or roll-your-own for purposes of Wyo. Stat. ' 9-4-1202.
2. Identify by name and address any other Manufacturer of any of the listed brands in the preceding or
current calendar year.
