Please complete the ENTIRE application. If a question is not applicable, answer N/A. DO NOT LEAVE THE LINE BLANK. TAX ATTESTATION (M.G.L. Chapter 62C, Sec, 49A), the tax attestation on page 8 of the application must be signed and dated as indicated. The following documentation must be submitted with your application: 1.
