ORDER TERMINATING SERVICES OF OUTSIDE CLAIMS AGENT
The above referenced debtor (the “Debtor”) filed a petition for relief under Chapter 11 with this court on
[Date]; the Clerk of Court having made a determination and having advised the Debtor that the case met the criteria
of a Large Asset Multiple Creditor case pursuant to 11 USC 156 (c); the Debtor having obtained an Order of
Retention on [Date] to retain [Name of the Claims Agent], to act as the Official Claims Agent of the Clerk of
Court; to assume full responsibility for noticing, processing of claims, preparation and maintenance of a claims
register and for custody of proofs of claims; the Plan having been confirmed on [Date], by order of this court; the
last date to file claims was [Date]; the Debtor, in consultation with the Clerk of Court, having determined that the
requirement for an outside claims agent no longer exists, it is hereby
ORDERED, that the services of [Name of the Claims Agent] are terminated effective thirty days from the
entry of this Order;
ORDERED, that [Name of the Claims Agent] will prepare final claims registers for the Clerk’s Office
pursuant to the Guidelines for Implementation of 28 USC § 156 (c);
ORDERED, that [Name of the Claims Agent] will box and transport claims to the Federal Archives, at
the direction of the Clerk’s Office pursuant to the Guidelines; and it is further
ORDERED, that the above services to be rendered by [Name of the Claims Agent] shall be a charge to the estate.
