Rule 13.0 Mandatory Forms to be Utilized in Criminal Cases in the State of Oklahoma
The following forms shall be utilized by trial courts and parties in the prosecution and appeal of criminal
cases in the State of Oklahoma. The forms may be computerized with additional lines and spaces added
as needed. In addition, appendices/ exhibits/attachments may be utilized to supplement the forms (for
example - rules and conditions of probation, payment schedule for fines and/or costs, etc.). Local
requirements may be added to the forms but the format and specific provisions in the forms shall not be
altered. The forms are as follows:
