TO THE CIRCUIT COURT:
You are hereby notified that the above named defendant will be represented in the above entitled

The defendant waives arraignment and the undersigned has advised the above named defendant
of the following rights:

plea.
It is requested that the defendant be released on his/her own recognizance. At least one week
prior to the Pretrial date the defendant shall go to the Release Assistant Office to be interviewed for
purposes of participating in a release decision.
