DEBTOR'S PROPOSED CHAPTER 11 PLAN OF REORGANIZATION
A disclosure statement under Chapter 11 of the Bankruptcy Code having been filed by
the Debtor, [name of Debtor(s)] (“Debtor”) on ____[date]_____ (“Debtor's Disclosure
Statement”), and which refers to a proposed Chapter 11 Plan of Reorganization filed by the
Debtor on ___[date]____ (“Debtor's proposed Chapter 11 Plan”), and in light of the Debtor's
election to be considered as a small business pursuant to §1121(e) of the Bankruptcy Code and
pursuant to the authority granted by 11 U.S.C. §1125(f)(3):
IT IS THEREFORE ORDERED, AND NOTICE IS HEREBY GIVEN, THAT:
A.
the Debtor's Disclosure Statement is CONDITIONALLY APPROVED;
B.
on or before _______________, a copy of this Order, the Debtor's proposed Chapter 11
Plan or a summary or summaries thereof approved by the Court, the Debtor's Disclosure
Statement as conditionally approved, and a ballot conforming to Official Form 14 shall
be mailed to creditors, equity security holders, and other parties in interest, and shall be
transmitted to the United States Trustee, as provided in Fed. R. Bankr. P. 3017(d);
however, in accordance with 11 U.S.C. §1126(f), acceptances and rejections will not be
solicited from any class that is not impaired under the proposed plan;

rejections of the Debtor's proposed Chapter 11 plan which must be received by 5:00 p.m.

deemed waived;
