PETITION BY DEFENDANT FOR OCCUPATIONAL DRIVER’S LICENSE
TO THE HONORABLE JUDGE OF SAID COURT:
Comes now ________________________________, hereinafter called Defendant/Petitioner, and respectfully
petitions the Court for an Occupational Driver’s License under the Provisions of Section 23A and 25, Article
6687b of Vernon’s Texas Civil Statutes, and would show unto the Court as follows:

Defendant/Petitioner’s driver’s license to operate a motor vehicle was suspended on the _______ day of

being attached hereto as Exhibit “A”. Said suspension was not because of physical or mental disability or
impairment.

The essential need for Defendant/Petitioner operating a motor vehicle in the performance of his/her

OCCUPATIONAL DRIVER’S LICENSE
Rules for County Court at Law, Fort Bend County, Texas, are as follows as Wit:
1. Verified Petition must have attached as exhibits:
a. copy of court judgment suspending driver’s license or DPS letter suspending driver’s license
b. affidavit of Defendant/Petitioner’s employer or supervisor as to type of work performed, hours
and place of employment, and necessity to drive a motor vehicle
c. proof of automobile liability insurance coverage - furnish original and two (2) copies of form
authenticated by an insurance company doing business in Texas and an insurance agent letter
stating date to which insurance is paid.
2. An information copy of the Petition is to be delivered to Criminal District Attorney in Fort Bend
County at time of filing.
3. Attorney for Defendant/Petitioner is to prepare conformed copies of Petition and Order to be certified
by clerk, and attorney will forward same to DPS.

It is further ordered that the Clerk of this Court shall mail to Texas Department of Public Safety the

3. Copy of Judgment of Conviction of this Court
4. Proof of liability of insurance in accordance with provisions of Texas Motor Vehicle Safety
Responsibility Act as provided in amended Article 670lh of Revised Civil Statutes of Texas by
furnishing a true and correct copy of Form SR-22
5. A certified copy of Defendant/Petitioner Petition for an occupational license with all exhibits
attached hereto
6. A certified copy of this Order
