A completed copy of this form will be returned to you along with the stamps ordered. An identification stamp must be
firmly affixed near the top of the brand label on the bottle of distilled spirits. Some portion of the identification stamp
should be attached to a portion of the brand label, but not to cover any information on the brand label. Each holder of
a Local Distributor’s permit shall prepare a record, making an entry on each date there occur transactions involving
identification stamps and show the following: (1) Quantity of identification stamps received; (2) Quantity and serial
numbers of identification stamps affixed; and (3) Quantity of identification stamps on hand after each receipt or affixing
of such stamps. Stamps must be applied only on sales to a Mixed Beverage permittee, Daily Temporary Mixed
Beverage permittee or Private Club permittee. This record must be maintained by the Local Distributor permittee on
their premises for a period of four (4) years for examination by any representative of this Commission. Upon
termination of any Local Distributor’s permit, all unaffixed identification stamps on hand must be surrendered to the
